Applying for DPH System Credentials
Download this form [PDF] and fill it out. Upload the completed form to https://bit.ly/UserRequest. It will take 5-7 business days to process. Once complete, credentials will be sent you you in a secure email.
You must complete the SFDPH online compliance training and e-sign the confidentiality and code of conduct agreements before logging on to a DPH system. Complete the training and sign the forms here: http://bit.ly/DPH_Compliance Instructions are included on the DPH User ID Request Form linked above.
Removing a user: If a previous user has left your organization, you may use the same form as above to delete their account. For deletions, we only need the first page completed and submitted.

To prepare for your transition to SFDPH CareLink on August 3rd, please take the time to review the attached training materials and tip sheets (3 total) and share with the rest of your staff.
A comprehensive guide which provides and overview of the functionalities within SFDPH CareLink. This will provide you with the steps on how to look-up a patient, search through a patient’s chart, place orders, and other useful items.
SFDPH CareLink users will be required to enter two-factor authentication before gaining access to the system. Review this tip sheet to see which options are available for two-factor authentication, and how to set-up your preferred method when you log-in for the first time.
SFDPH CareLink users will be able to submit eConsults/eReferrals through SFDPH CareLink. This tip sheet provides the step-by-step process on how to submit an eConsult/eReferral, how to respond to a specialist, and how to review eConsults/eRefferals that have been submitted for patients.
Training materials and tip sheets will also be available within SFDPH CareLink – Quick Links section on the home page once a user logins for easy reference.
User accounts for SFDPH CareLink are in the process of being created. Please keep an eye out in the next week or so about your site’s user information.
Commonly Asked Questions
I am having trouble with my password. What can I do?
If you have forgotten your password or if your account has been locked out because you tried logging on too many times, you can call the SFDPH Help Desk at (628) 206-7378 and ask for a password reset. After they change your password, you MUST follow the procedure below to change it the first time.
Your password will automatically expire and must be changed every 90 days. You will not be able to log on to Epic Carelink with an expired password. To change your password, follow these steps:
Log on to https://webconnect.sfdph.org/sfccc-duo with your expired password.
If your password ha expired, you will be prompted to change the password before you can continue.
Once your password has been changed, you will be asked to log on again to Webconnect.
If you are able to log on to Webconnect, you can log out and use your NEW password to log on to Epic Carelink.
If you continue to have problems logging on to Webconnect or Epic Carelink, call the SFDPH Help Desk at (628) 206-7378.
Is there any required online training for SFDPH CareLink?
No, there is no required online training. Training for SFDPH CareLink will consist of a Quick Start Guide and some tip sheets. These training materials will also be available to users once they login to the home page of SFDPH CareLink.
How will we access SFDPH CareLink?
SFDPH CareLink will be a public facing website that can be accessed anywhere as longer as the user has access to internet.
The URL for SFDPH CareLink will be shared with users once our production site is up.
When can we start using SFDPH CareLink?
Users can access SFDPH CareLink starting on August 3, 2019.
What if I have users at my site who also works at a DPH (ZSFG, LHH, etc.) that is going live with full Epic access on August 3rd?
Users who also work at a DPH site that is going live with full Epic access on August 3, 2019 will only be given access to Epic and not SFDPH CareLink.

Instructions on how to log on to the learning site will be sent along with your eCW credentials. Contact email@example.com if you have not received those instructions.
Within LCR, DPH offers access to a service that consolidates historical information on patients from a number of sources. CCMS (Coordianted Care Management System) pulls information from Emergency Room, Ambulance, Housing, Behavioral Health and Jail data sources to provide a comprehensive view of at-risk persons for the care provider.

Applying for Avatar credentials
Avatar User Credentials are issued by applying directly to DPH. For the forms, please visit the Avatar Documentation Page on SFDPH website (https://www.sfdph.org/dph/comupg/oservices/mentalHlth/BHIS/avatarUserDocs.asp ). Along with the on-boarding forms, this site has access to instructional videos and additional resources.
Everyone is required to attend training before getting their login information. Watching the instructional videos is insufficient to meet this requirement.
Everyone is required to submit a certification form that includes Taxonomy Code and NPI number. Staff will need to go the NPI Registry (https://nppes.cms.hhs.gov/ ) to register and have an NPI number assigned to them. The certification form is not considered complete without the NPI/Taxonomy.
Each form has a fax number on it (Compliance 415.252.3032 / Avatar Accounts 415.252.3008). Compliance receives the certification form and electronic signature agreement. Avatar Accounts receives the account request form. It takes about 10 business days to complete the process (including creation of the Webconnect account if needed). Account information is usually emailed to the requesting supervisor.
