Continuous Quality Improvement
Our network-based Continuous Quality Improvement program addresses clinical outcomes of SFCCC-sponsored programs, targeting specific conditions for improvement on a network level. CQI is overseen by a committee that is appointed by the board each year, and is both government and privately funded.
Quality Improvement Committee
Quality Improvement Committee meets on a monthly basis at SFCCC headquarters or participant member clinic. The QIC is a key participator in development of health center’s QI Plan. It works closely with Medical Directors and other Health Center QI Staff on health center’s QI activities, which may include periodical assessment of health center performance on select indicators and implementation of changes through quality improvement initiative.
Quality Performance Measures
The San Francisco Community Clinic Consortium and its member clinics are committed to quality improvement to promote better clinical outcomes and deliver high quality service. The SFCCC network of ten Community Clinics and Health Centers serve over 93,000 low-income, uninsured, and medically underserved people each year.
