San Francisco’s Critical Health Care Player

San Francisco has an amazing health maintenance organization, HMO, the San Francisco Health Plan (SFHP).  SFHP celebrates 15 years of service to our community this year.  Over 16 years ago, the State of California announced that it planned to transition patients using Medi Cal as their insurer from a fee for service system to a managed care system.  SFCCC, the Hospital Council of N. CA, SF Medical Society and the SF Department of Public Health immediately started working together to create a locally governed HMO that would protect the lives of our residents and improve the health of our patients and communities.  Today, 15 years later, the plan is going strong with over 60,000 patients enrolled.  Offering medical homes for Medi Cal, Healthy Families, Healthy Kids, IHSS Workers, SFHP annually excels in data that demonstrates top level outcomes in immunization, pap smear, hypertension screening, diabetes care and much more.  SFCCC is proud to be a founder and partner with SFHP.  On behalf of the Board of Directors of SFCCC and our ten Partner Health, we wish you a Happy 15th Year Anniversary.
