A: The Street Outreach Services (SOS) Van,

A: VET SOS, or Veterinary Street Outreach
Services, is a project of SOS, designed to expand outreach to homeless
San Franciscans, by offering veterinary care to their companion animals.

Q: What is a medical home?
A: A medical home is not simply a place, but a model of primary care that delivers care that is patient-centered, comprehensive, coordinated, accessible and continuously improved through a systems-based approach to quality and safety.
Q: What is a health care "safety net"?
A: A health care safety net consists of those providers that organize and deliver a significant level of health care and other health-related services to uninsured, Medicaid, and other vulnerable patients. The current San Francisco health care "safety net" delivery system for the uninsured and underinsured consists largely of the SFCCC and SFDPH provider networks. Healthy San Francisco (HSF) is an innovative local program that links uninsured residents to a primary care medical home. More than 90% of the uninsured residents who are served by HSF have an SFDPH or SFCCC clinic as their assigned medical home.
