Job Description:

The Mission Neighborhood Health Center (MNHC) is a non-profit, multi-service health care facility offering comprehensive health services for ambulatory patients.  The center offers primary health care services, adult medicine, pediatrics, family planning, OB/Gyn, and HIV Services.  The Center is committed to high quality, culturally competent primary care services, focusing on the Latino/Spanish-speaking population.

 

The Excelsior Clinic, located in the Excelsior district, is a smaller satellite clinic that provides pediatric, prenatal, womenfs health and adult medicine services.  We are seeking an NP who wants to make a real difference by helping those who are most in need. 

 

General Statement of Duties & Responsibilities:

The nurse practitioner is responsible for the triage of all patients and will supervise all medical assistants and nursing staff in establishing a clinical team approach to comprehensive medical care. This position may allow for up to 20% time continuity clinics and 10% administrative time.

 

Reporting Relationships:

Reports to and is directly responsible to the Director of Nursing or designee for administrative and clinic operations as it relates to functions improvements and reports directly to the clinical chief of the Excelsior as it relates to progress of process improvements.

 

Responsibilities:

  • Effectively triages patients with appropriate attention to acuity and the availability of medical providers for urgent care evaluation. Medically attends to those patients whose presenting problems are comfortably within employee skill level.
  • Assists in the evaluation of women seeking birth control.
  • Provides medical treatment as ordered and performs procedures according to MNHC nursing protocol.
  • Provides leadership, evaluations, and direct supervision to the medical assistants and support staff.
  • Supervises the patient flow in clinical area with particular attention to the appropriate triage as well as the effective communication with other staff that impact patient flow such as registration, medical records, and referral coordinator staff.
  • Supervises the preparation of patients for medical evaluation including an assessment that patients are correctly vitalized and monitors availability of charts, labs, and forms.
  • Supervises the adequate stocking of the patient rooms/department and efficient re-ordering of materials
  • Supervises the disposition of patients after medical evaluation to ensure that the appropriate medical plans are properly enacted including imparting the relevant health education material.
  • Supervises the effective participation in Quality Improvement Activities with particular attention to the programs and registries for chronic diseases and reports to the QI committee
  • Monitors all the logs including the Pap, Mammo, PPD, Pre-natal,     and Vaccine logs.
  • Performs other duties as assigned by Director of Nursing/Medical Director
  • Attends and participates in nursing staff meetings.

Qualifications:

Current California licensure as a Nurse Practitioner.

Bilingual Spanish/English.

Available to work extended hours

Exposure to community health nursing.

Knowledge and understanding of Latino communities/culture preferable.
