The Mission Neighborhood Resource Center is an innovative service of the Mission Neighborhood Health Center in collaboration with North Mission service providers, community advocates, residents and merchants to address the needs of homeless and marginally housed communities in the 16th Street Corridor.

The Resource Center’s integrated services provides health and dental care, acupuncture, substance abuse, mental health counseling and peer-led support as well as access to basic amenities such as bathrooms, showers, lockers, and laundry facilities.  The Resource Center provides participant-centered, harm reduction services to improve the health and well being of clients by assisting them to engage in programs, reduce harms, and ultimately achieve self-determination, financial stability and independence. 

GENERAL STATEMENT OF DUTIES AND RESPONSIBILITIES;

Under the direct supervision of the MNRC Medical Director and as a member of an integrated health and social services team, the Family Nurse Practitioner will deliver primary and urgent care to a multicultural, multilingual and very diverse community of patients dealing with homelessness, substance abuse, mental health issues and HIV status.

The Family Nurse Practitioner is responsible for the primary and urgent health care of a diverse patient population including day laborers, substance users, chronically mentally ill, transgender individuals and street sex workers.

REPORTING RELATIONSHIP:

Reports directly to the Mission Neighborhood Resource Center’s Medical Director.

SAMPLE JOB DUTIES:

• Responsible for the primary and urgent health care of a diverse patient  population including day laborers, substance users, chronically mentally ill, transgender and sex workers. Includes management of common acute and chronic conditions, soft tissue infections, transgender hormone therapy, sexually transmitted infections, well women care and common gynecological complaints.
• Provide harm-reduction centered health education, preventive care strategies and counseling to patients.
• Collaborate with multidisciplinary team including case management, therapists, and psychiatry.
• Maintains continuity of care for patients by making referrals to community agencies, and providing follow-up for recently hospitalized and referred patients.
• Maintain medical records, participate in quality assurance activities.

QUALIFICATIONS:

• Valid Nurse Practitioner license and furnishing license.
• Valid California RN license.
• Minimum of one year experience as Nurse Practitioner.  Experience providing care to homeless populations with multiple diagnoses a plus.
• Bilingual English/Spanish required.

The Center is located 2 blocks away from the 16th Street/Mission BART station and is accessible via MUNI lines 14, 22, 33 and 49.
