Job Description:

Curry Senior Center, a Tenderloin based non-profit geriatric health and social services agency serving low-income older adults, seeks an experienced Case Manager. An integral component of Curry Senior Center’s services (which include primary medical care, substance abuse treatment, mental health services, housing, community programs and congregate meals), the goal of case management is to facilitate the provision of necessary services to enable seniors to function independently in the community.

 

Primary Responsibilities:

  • Under the supervision of the Case Management Supervisor (LCSW), the case Manager works with a caseload of frail elderly 55 years and older who are homeless or at risk of homelessness.
  • The Case Manager will conduct functional assessments (including in-home assessments); care planning and ongoing coordination of support services, done in collaboration with multi-disciplinary team; service authorization and advocacy; case monitoring, including follow-up and reassessment of needs; assist elders with housing readiness, advocate for housing, health  care services and entitlements; recordkeeping (written documentation, following standard chart protocols and statistics); participate in staff meetings, multi-disciplinary case conferences, and in-service trainings; and other duties as required.

Minimum requirements:

  • BA degree in social work or other health/human services field; or AA degree with course work in case management or related field.  Bilingual and bicultural preferred.
  • Must be willing to work in a community based setting in the Tenderloin neighborhood with low income, frail, and culturally diverse older adults (including non English speakers, homeless) and individuals with multiple behavioral and physical health problems.
  • Must be willing and able to do home visits in the Tenderloin – South of Market neighborhoods and other neighborhoods where senior live.
  • Ability to maintain the confidentiality of sensitive information and comply with all HIPPA regulations
  • Willingness to report for duty in an emergency disaster.

Compensation:  Salary competitive with other non-profit organizations providing similar services.
