You are cordially invited to the San Francisco Community Clinic Consortium 2014 Gala. We will be honoring Mayor Ed Lee for his leadership and innovation in a client-centered approach to healthcare; for San Francisco’s thorough implementation of the Affordable Care Act; for the City’s new approaches to healthcare and housing for homeless people; and for continuing the important Healthy San Francisco program.

SFCCC was formed thirty years ago out of a mix of vision and necessity. It was 1982, health care costs were rising at an alarming rate and government funding was shifting away from community clinics. For the clinic leaders who founded SFCCC, these changes — and others that loomed large on the horizon — signaled crisis. The very existence of the clinics they had fought so hard to establish, and their ability to provide access to care for uninsured and low-income community members, were at stake. It was time to come together — strengthened in partnership — to face the challenges that lie ahead.

SFCCC shares with its partner clinics a vision of equal access to health care. With a day-to-day focus on patient care, our partners promote the health of the individual to preserve the well-being of the communities they serve. SFCCC focuses on the “big picture” of an ever-changing and competitive health care environment. Our charge is to promote the well-being of our partners, so that their long-standing tradition of community-based health care is preserved.

The SFCCC partnership provides primary care services to more than 91,100 San Franciscans each year — from infants to elders, and everyone in between. We work toward a future in which all people have access to quality, community-based health care provided in a culturally, linguistically and population- sensitive manner.

Mission Statement

The San Francisco Community Clinic Consortium, a partnership of not-for-profit primary care health centers, develops programs and advocates for policies that increase access to community-based primary care for all San Franciscans, targeting the uninsured and under served.
