SFCCC Partner Clinics Serve the Majority of Healthy San Francisco Participants

As of June 30, 2012, San Francisco Community Clinic Consortium partner clinics served as primary medical care homes for 25,741 of Healthy San Francisco’s 46,822 enrollees, or 55% of all HSF program participants.  NEMS alone serves more than 30% of the HSF patient population.  Other SFCCC partners -- Mission Neighborhood Health Center, Haight Ashbury Free Clinics – Walden House, South of Market Health Center, Saint Anthony Medical Clinic, Glide Health Services, Lyon-Martin Health Services, and Native American Health Center -- collectively serve nearly another quarter of HSF participants. 

Primary care services that are provided by SFCCC partners to HSF patients include preventive care, such as immunizations; treatment for acute medical conditions; chronic disease management, e.g., for diabetes, asthma; diagnostic lab services; and health education.  HSF patients are also eligible for additional services, such as pharmacy, specialty care, hospital services.  For more information about HSF, please visit http://www.healthysanfrancisco.org/ or call the City Information line at 3-1-1 or (415) 701-2311 outside of San Francisco.

June 30, 2012 marks the five-year anniversary of the HSF program. SFCCC clinics are proud of their significant role to date as HSF providers, and look forward to their continued participation in serving uninsured San Franciscans.
